All chapters must complete the online Chapter Affiliation Application (https://www.cal-hosa.org/affiliation-survey/) as the first step in the affiliation process.

Once the approval is granted, advisors can move to step two – online chapter affiliation at hosa.org. Chapters must be in good standing to affiliate! 

Cost to affiliate with HOSA will be:

• Advisors = $25       ($10 – HOSA FHP,              $15 – Cal-HOSA)
• Students = $20       ($10 – HOSA FHP,             $10 – Cal-HOSA )
